Recurring Charge Agreement
By accepting this Agreement you also authorize if applicable an automatic monthly payment of your HealthSTYLE invoice from the credit card provided. Visa and MasterCard accepted. Please verify that all your information is complete and accurate before placing order. The monthly charge will appear as Welnia on your statement. The effective date of your agreement becomes the monthly credit card charge date.
Recurring Payment Failure
If your recurring payment is declined, your Recurring Credit Payment Option will be removed and we may restrict your ability to use the payment method that was declined (e.g., credit card) for a period of time. We may permanently restrict your ability to use a certain payment method if that payment method fails multiple times. You are still responsible for finding an alternate card or method of payment or services may be interrupted.

Termination of Service Agreement
30 days written notice is required to terminate your service agreement. The termination shall be effective on your first billing date following the expiration of the thirty (30) day period. If you terminate within the first 6 months, additional termination fees may apply according to the following schedule:

Month 1: $375.00

Month 2: $ 250.00

Month 3: $ 125.00

Month 4: $ 75.00

Updating Payment Options
You may at any time change your payment methods by contacting Customer Service at 866-493-5642.

Coaching Requirements
You are requested to give 24 hours notice if you need to cancel your scheduled coaching session to avoid losing the time slot for the designated period.
